01.What a plan of care includes
Every plan has the same backbone: a clinical summary of the evaluation, long-term goals (where we're heading in 6โ12 months), short-term objectives (steps to get there), recommended frequency and duration of sessions, and a discharge plan describing what 'done' looks like.
02.How goals are written
Good pediatric therapy goals are functional and measurable. Instead of 'improve fine motor skills,' a goal might read: 'Within 12 weeks, child will independently zip a coat in 4 of 5 trials across 3 consecutive sessions.' Functional goals tie directly to your real life โ getting dressed, eating with peers, joining a game on the playground.
03.Frequency and duration
Your therapist recommends how often (e.g., 1โ3x per week) and how long (e.g., 45โ60 minutes) based on your child's needs and the research. Consistency matters โ regular attendance leads to faster, more lasting progress, while frequent gaps slow things down. We'll always work with your schedule and adjust the plan as your child grows and improves.
04.Reviewing & updating the plan
Plans of care are formally reviewed at least every 6 months, and sooner if needed. Your therapist tracks progress on every goal, and we adjust intensity, swap goals, or transition you to a new phase of treatment as your child meets benchmarks.
05.Your role as the parent
You don't just receive the plan โ you co-author it. Tell us what matters most to your family. Share what's working at home and what isn't. Try the home strategies we send and let us know how they land. The most successful plans are the ones built with parents, not at them.
When goals change
It's normal โ and a sign of progress โ when goals are met or rewritten. If you ever feel a goal no longer fits your family's priorities, say so. We'd rather adjust than coast on a goal that's lost relevance.

Can I see my child's plan of care?+
Absolutely. You'll receive a copy after the evaluation and at every formal review. Ask anytime for an updated copy.
What if I disagree with the plan?+
Tell us. The plan should reflect your priorities โ if a goal feels off, we'll talk through the clinical reasoning and adjust where it makes sense.
How do you measure progress?+
Therapists collect data every session on the goals in your plan. We share progress at every review and can give updates anytime you want one.
